From History of Washington County, Pennsylvania

     "Robert Doak came from Harrisburg before 1780 and settled on land which was surveyed June 2, 1785, containing 400 acres, named "Doak Plain." Robert and his son William lived on the farm till their death (Robert died 1803; Wm. died 1867). He used to relate that they were driven from the place several times by the Indians, and once remained away two years."

     "FRANKLIN DOAK, born in 1840, a Civil War soldier, stated in a conversation, at Brownsville, Pa., on June 6, 1928, that his father, Robert Doak, had five children, Jack, John, Julia, Sarah, Franklin; and that his father's brother, Joseph, had a son William, whose son was named Robert. The old soldier's grandfather's name was Robert."

CLAIMANT'S AFFIDAVIT
State of Pennsylvania, County of Fayette, ss:
In the matter of claim No. 1.015.994 for pension of Franklin Doak, late Corporal Co "B" First Va. Vet. Mounted Volunteers.
On this third day of August AD 1891; personally appeared before me a Notary Public in and for the aforesaid County duly authorized to administer oaths, FRANKLIN DOAK the claimant aged 51 years, a resident of Brownsville in the County of Fayette and State of Pennsylvania whose Post Office address is Brownsville, Fayette County, Penna...... well know to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows:
     I received two wounds (gun shot) in the left arm on the 3rd day of May 1863 at Worrington Junction, on the Alexandria and Orange Rail Road in Virginia. I was on picket duty at the time and I received the two shots in left arm while resisting a charge by the enemy.
     I have not been in the military or naval service of the United States since July 8" 1865, at which time I was discharged from Co "B" First Va. Vet. Mounted Volunteers.
     I enlisted first as private in Capt H. P. Boons Co "B" First Regt. West Va. Cavalry, on the six day of August 1861 and was discharged on the twenty eighth day of February 1864, at Wheeling West Virginia by reason of reinlistment, and was remustered as a veteran.
     I was enrolled on the Fourth day of March 1864, as Corporal in Capt Hugh P. Boons Co "B" First Regt. West Va. Vet. Mounted Volunteers, and was discharged on the eighth day of July 1865 (as above stated) by reason of instructions from War Department. I have both discharges in my possession, and the dates of term of service is taken from there.

Declaration for Pension

     On this 20 day of May, A.D. one thousand nine hundred and 12 personally appeared before me, a NOTARY PUBLIC within and for the county and State aforesaid, FRANKLIN DOAK, who being duly sworn according to law, declares that he is 71 years of age, and a resident of SOUTH BROWNSVILLE, county of FAYETTE State of PENNSYLVANIA; and that he is the identical person who was enrolled at Wheeling WVa under the name of FRANKLIN DOAK on the 6 day of AUGUST, 1861, as a private, in Co B 1st Reg W.Va. Vol. Cal corporal W.Va. vet mounted vol. in the service of the United States, in the Civil war, and was honorably discharged at Wheeling W.Va. on the 8 day of July, 1865.
That he also served - none.
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 10 inches; complexion, SANDY; color of eyes, BLUE; color of hair, SANDY; that his occupation was FARMER; that he was born NOVEMBER 8, 1840.
That his several places of residence since leaving the service have been as follows: SOUTH BROWNSVILLE, PENNSYLVANIA
That he is a pensioner under certificate No. 753105.
That his post office address is SOUTH BROWNSVILLE, county of FAYETTE State of PENNSYLVANIA.

Declaration for Pension

State of Pennsylvania, County of Fayette
     On this 22nd day of Sept, A.D. one thousand nine hundred and sixteen, personally appeared before me, a Justice of the Peace within and for the county and State aforesaid, FRANKLIN DOAK, who, being duly sworn according to law, declares that he is 75 years of age, and a resident of SOUTH BROWNSVILLE county of FAYETTE State of PENNSYLVANIA; and that he is the identical person who was enrolled at WHEELING W.VA., under the name of FRANKLIN DOAK on the 6th day of AUGUST, 1861, as a PRIVATE, in COMPANY B - 1st REGIMENT OF WEST VA CAVALRY VOLUNTEERS in the service of the United States, in the -Civil- war, and was honorably discharged at Wheeling, W.Va. on the 28th day of February, 1864.
That he also served as Corporal in Company B Regiment of West Virginia Vet Mounted Volunteers, on the 4th day of March 1864 and was discharged on July 8th 1865 at WHEELING, W.VA.
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 10 inches; complexion, SANDY; color of eyes, BLUE; color of hair, SANDY; that his occupation was FARMER - that he was born NOVEMBER 8th, 1840, at WASHINGTON, WASHINGTON COUNTY, PENNSYLVANIA.
This his several places of residence since leaving the service have been as follows: WASHINGTON PA, BRIDGEPORT PA, now SOUTH BROWNSVILLE, FAYETTE Co PA.
That he is a pensioner under certificate No. 753105. That he has -- applied for pension under original No. 753105.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.
That his post-office address is SOUTH BROWNSVILLE, county of FAYETTE, State of PENNSYLVANIA.
