and M.O. June 16th, 1865.
From En. to m.o. he held the rank of private and during the period the rolls show
him present except as follows June 30/63, absent. missing since engagemet at Winchester,
Va. June 15/63. Aug. 31.63, same report. June 30/4, absent detached in Pioneer Corps
since May 1.64. Aug.31/64. absent without leave since Aug. 15/64.
Prisoner of War Records show him captured at Winchester, Va. June 15/63. Paroled at
City Point, Va. July 2/63; reported at Camp Parole, Md July 3/63; On list of
deserters from Camp Parole Md. July 10/63 to Aug 1/63.
Designation changed May or June 1863 to Co. D. 12 W.Va. Inft.
The medical records show him treated as follows: as G. D. Teagarden, Priv. Do. D, 12 Va Inf. July 4 to 11.63. Diarrhrea returned to duty: as G. D. Teagarden, Priv. Co D. 12 Va. until July 14, 63, Stomatitis. Nothing additional found.

A DECLARATION FOR INVALID PENSION.
Note - This is soldier's Application and must be executed with any two persons, soldiers not required, before any officer authorized to administer oaths for general purposes. If such officer is required to use a seal, he must affix the same to his jurat or paper will not be accepted. If he is not required to use a seal, a certificate of his official character, under the hand and seal of a clerk of court or other proper officer must be attached. COUNTY OF LA PLATA
On this Apr 26 1897 day of A. D. one thousand eight hundred and ninety-seven personally appeared before me clerk and recorder within and for the county and State aforesaid George H. Teagarden aged 55 years, a resident of the city of DURANGO, county of LA PLATA, state of COLORADO who, being duly sworn according to law, declares that he is the identical person who was ENROLLED on the 27th day of August, 1862, in Co. D. 12th W. Va. Inf. in the war of the rebellion, and served at least ninety days, and was HONORABLE DISCHARGED at Richmond Va. on the 16th day of June, 1865. That he has not been in any military or naval service U.S. since then. That he is totally unable to earn a support by reason of disabilities. Viz; Piles, Disease of Liver, Disease of Stomach and Rheumatism and that said disability existed in a pensionable degree June 27, 1890. That he makes this application for Re-issue of Pension under act of March 6, 1896, which provides:
"That whenever a claim for pension under act of June 27, 1890 has been or shall hereafter be, rejected, suspended, or dismissed, and a new application shall have been, or shall hereafter be, filed, and a pension has been, or shall hereafter be, allowed in such claim, such pension shall date from the time of filing the first application, provided the evidence in the case shall show a pensionable disability to have existed, or to exist, at the time of filing such first application, anything in any law or ruling of the Department to the contrary notwithstanding."
That his disabilities are not due to vicious habits, and are to the best of his knowledge and belief permanent. That he has applied for pension under application No. (blank) That he a pensioner under Certificate No 904543
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provision of the act of June 1890
(If a pensioner, the Certificate number only need be given. If not, give the number of the former application, if one was made).

Declaration for Increase of Invalid Pension.
State of Colorado
County of La Plata
On this 26th day of May A.D. one thousand eight hundred and ninety-six personally appeared before me, County Clerk within and for the county and state aforesaid, George H. Teagarden
aged 54 years, a resident of Durango, county of La Plata, state of Colordado, who, being duly sworn according to law, declares that that he is the identical George H. Teagarden who was enrolled on the 14th day of August, 1862, in "D" 12th Regt West Va Infty Vols in the War of the Rebellion and served at least ninety days, and was honorably discharged at Richmond Virginia on the 16th day of June, 1865.
That he is now unable to earn a support by reason of chronic piles, rheumatism,
catarrh of stomach & head, neuralgia, and dispepsia, and frozen feet, lumbago, the he has not served in the army or navy of the U.S. since June 16th, 1865.
That said disablities are not due to vicious habits, and are to the best of his knowledge and belief permanent. That he has applied for pension under application No. ____. That he is now a pensioner under certificate No. 904,543
That he makes this declaration for the purpose of being placed on the pension-roll of the United States under the provisions of the Act of June, 1890. at an increased rating, because of increased disability.
He hereby appoints W. C. Davidson, ATTORNEY AT LAW, DURANGO, COLORADO,
his true and lawful attorney to prosecute his claim. That his post-office address is Durango

On this 27th day of December, A. D. one thousand eight hundred and ninety-three, personally appeared before me, J. P. Ainy County Clerk within and for the County and State
aforesaid George H. Teagarden aged 51 years, a resident of the City of Durango County of La Plata, State of Col. who, being duly sworn according to law, declares that he is the identical
Geo. H. Teagarden who was ENROLLED on the 14 day of Aug. 1862, in Co. D. 12 W. Va Inf.
in the war of the rebellion, and served at least ninety days, and HONORABLY DISCHARGED at Richmond Va., on the 16th, day of June, 1865. That he has not been employed in the military or naval service otherwise than as stated above.
That he is greatly unable to earn a support by manual labor by reason of catarrh in head,
dyspepsia, neuralgia, dizziness, disease of kidneys, lumbago, frozen feet, general
debility, piles and rheumatism. That said disabilities are not due to his vicious habits, and are, to the best of his knowledge and belief, permanent. That he has applied for pension under application No. 1095219. That he is a pensioner under certificate No. ________
That he makes this declaration for the purposes of being placed on the pension roll of the United States, under the provisions of the Act of June 27, 1890.
He hereby appoints, with full power of substitution and revocation, JAMES TANNER, of Washington, D.C., his true and lawful attorney, to prosecute his claim, and in consideration of services done, and to be done, in the premises, he hereby agrees to allow his said Attorney, JAMES TANNER, a fee of ten dollars, payable only in the event of the allowance of the claim by the Commissioner of Pensions.

ACT OF MAY 11, 1912
DECLARATION FOR PENSION
THE PENSION CERTIFICATE SHOULD NOT BE FORWARDED WITH THE APPLICATION.
State of Colorado, County of Moffat, SS:
On this 6th day of July, A.D. one thousand nine hundred and twelve, personally
appeared before me, a notary public within and for the county and State aforesaid,
George H Teagarden who, being duly sworn according to law, declares that he is 70
years of age, and a resident of Pagoda, county of Moffat State of Colorado; and that he is the identical person who ENROLLED at Wheeling West Va, under the name of George H. Teagarden,
on the 14th day of August, 1862, as a private, in Co D 12th Regt West Va Inft in the service of the United States, in the Civil war, and was HONORABLY DISCHARGED Richmond Va, on the 29th day of May, 1865.
That he also served no other service.
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height 5 feet 8 inches; complexion, dark; color of eyes, dark; color of hear, dark; that his occupation was Farmer; that he was born Washington Co Pa Jan 7th, 1842, at Washington Co Pa Jan 7th 1842.
That his several places of residence since leaving the serivce have been as follows: after discharge in Ill. 4 years, in Iowa 2 years, Texas 18 months, came to Colorado
1874 and have remained there since.
That he is a pensioner under certificate 904543 Special Act of Congress. That he has applied for pension under original No. ____.
That he is not unfit for manual labor from dyspepsia, Rheumatism.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.
That his post-office address is Pagoda, county of Moffat State of Colorado
