State of OHIO, County of KNOX, ss:
On this 1st day of FEBRUARY A.D. one thousand eight hundred and eighty EIGHT personally appeared before me a Clerk of the Common Pleas Court, a Court of record within and for the county and State aforesaid JAMES W. COE age 43 years, who, being duly sworn according to law, declares that he is the identical JAMES. W. COE who was enrolled on the 12th day of AUGUST, 1862 in Company A of the 12th regiment of West Va Infty commanded by Capt. H. Tumbleson and was honorably discharged at Richmond Va on the 16th day of June 1865. That his personal description is as follows: Age 18 years; height 5 feet 6 inches; complexion DARK, hair, DARK; eyes, BLUE. That while a member of the organization aforesaid, in the service and in the line of duty at BELLE ISLE in the State of Virginia on or about JUNE & JULY, 1863, he being a PRISONER OF WAR and having to eat bad and improper food and not enough of even that contracted Dyspepsia or disease of the stomach causing general debility and disease of the heart, from all of which he has suffered to the present time. That his exposure on Belle Isle to bad weather without blankets or proper protection greatly aggravated his disability - Sometime in July 1863 he was paroled and sent to Camp Parole at Annapolis, Md for about two weeks when broke guard and went home where he lay sick for several weeks and then reported to Wheeling Va. from which place he was sent to Camp Chase Ohio until November when he was sent to his Co at Harpers Ferry Va.
That he was treated in hospitals as follows: Not at all and had no treatment by his Regiment Surgeon except two or three times.
That he has not been employed in the military or naval service otherwise than as stated above.
That he has not been in the military or naval service of the United States since the 16th day of June 1865.
That since leaving the service this applicant has resided in the Counties of COSHOCTON & KNOX in the state of OHIO and about a year in WEST VA and that his occupation has been that of FARMER.
That prior to his entry into the srvice above named he was a man of good, sound, physical health, being when enrolled a FARMER.

Sir:
I have the honor to request that you will furnish from the records of the War Department a full Report as to the service, disability, and hospital teatment of JAMES W. COE, who, it is claimed, enlisted AUG 12, 1862, and served as PRIVATE in Co. A. 12 Reg't W.VA. VOLS; also in Co. by Capt. H. Tumbleson and was discharged at Richmond Va June 16, 1865.
While serving in Co. A 12 Reg't W.Va. Vols he was disabled by Dyspepsia, affection of stomach and general debility while a prisoner of war at Belle Isle Va. June & July 1863. Also Disease of Heart same time & place.

May & June/63, absent - missing in action at Winchester Va June 13/63 - (Co was in action June 13/63 at Winchester Va)
July & Augt/63 Same.
Sept & Oct/63 same with remark: EX PRIS OF WAR at Camp Chase Ohio
Nov & Dec/63. Present, same to April 30/65

He was "Captured in action at Winchester, Va. June 13, 1863; confined at Richmond, Va.; June 23, 1863, and paroled at City Point, Va. July 8, 1863. Reported at College Green Barracks, Md, July 9, 1863 and sent to Camp Parole, Md. July 11, 1863 where he arrived the same day. Reported at Camp Chase, Ohio, Sept 10" 1863 and sent to regiment between Oct 20 & 31, 1863."
Declaration fo Widow's Pension
State of Ohio, County of KNOX, ss:
On this 17th day of JANUARY, A.D. one thousand nine hundred and TWELVE personally appeared before me, a Notary Public with and for the county and State aforesaid SARAH C. COE aged 64 years, a resident of MT VERNON, County of KNOX State of OHIO, who, being duly sworn according to law, declares that she is the widow of JAMES W. COE DECEASED, who enlisted under the name of James W Coe on the 12th day of August 1862, in Co A 12th WVa Infty A PRIVATE and served at least ninety days in the late War of the Rebellion, in the service of the United States, was Honorably Discharged and died JANUARY 12th 1912.
That she was married under the name of SARAH C. FRY to said JAMES W. COE on the 16th day of AUGUST, 1868 by Isreal Dillon, at East Union, there being no legal barrier to said marriage...
That she has not remarried since the death of the said JAMES W. COE.
(No children under sixteen.)
That her husband has NOT been employed in the Military or Naval service otherwise than as stated above.

Dear Sir:
I am now drawing a pension under Certificate numbered 737171 on account of the service of the Soldier mentioned therein.
My soldier husband served in the lat war of the rebellion under the name of James W. Coe. he entered the service August 12th 1862 and was discharged June 16th 1865. Served in Co. A. 12th regt. W.Va. Inf.
I was married to said soldier August 16th 1868 at Coshocton, Ohio. My maiden name was SARAH CATHERINE FRY. I am now 70 years of age. I was born June 27th 1847 in Coshocton County, Ohio.
I hereby make application for an increase in my pension as allowed by act of Congress of September 8th 1916.
My postoffice address is No 113 East Hamtramick St. Mt Vernon Ohio.
