On this 30th day of September A.D. one thousand nine hundred and twelve personally appeared before me, Notary Public within and for the County and State aforesaid, FRANCIS M. SOCKMAN, who, being duly sworn according to law, declares that he is 68 years of age, and a resident of County of WASHINGTON, State of PENNSYLVANIA; and that he is the identical person who enrolled at - - under the name of FRANCIS M. SOCKMAN, on the 27th day of February, 1864, as a Private, in Co. C. 12th Regt. W.Va. Inf. Vols. in the service of the United States, in the Civil War, and was Honorably Discharged at Fortress Monroe, Va., on the 22nd of July, 1865.
That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 9 inches; complexion, FAIR; color of eyes, DARK; color of hair, SANDY; that his occupation was FARMER; that he was born AUGUST 9th, 1844 at MARSHALL CO. VA.
That he is a pensioner under Certificate No 71422.
That his Post-Office address is 240 Forrest Ave, County of Washington, State of Pennsylvania

In the matter of claim for pension of FRANCIS M. SOCKMAN Co C. 12th Regt. W.Va. Inf. Vols. No 71.422
On this 2 day of MAY, A.D. 1913, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths FRANCIS M. SOCKMAN aged 68 years, a resident of WASHINGTON in the County of WASHINGTON and State of PENNSYLVANIA whose Post Office address is 240 FORREST AVE, WASHINGTON, PA well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to the aforesaid case as follows:
That he is claimant in the case, that he is unable to furnish a certified copy of the public or church record of the date of his birth, because no such records were ever made - That he is unable to furnish a certified copy of the family record of the date of his birth, for the reason that the family Bible containing said record is lost or destroyed. That he has often seen paid record and he knows that it set forth the fact that he was born on the 9th day of August 1844 and his mother has often told him that he was born on that date and this is the best obtainable evidence of the date of his birth he can furnish.

On this 5th day of February, A.D. 1938 before me, the undersigned, personally appeared WYLIE A. SOCKMAN, aged 54 years, a resident of FERNDALE, County of OAKLAND, State of MICHIGAN, who makes the following declaration as an application for, and claim is hereby made for, reimbursement from the accrued pension for expenses paid (or obligation incurred) in the last sickness and burial of FRANCIS M. SOCKMAN, who was a pensioner of the United States by Certificate No. 71422, and who died January 29, 1938, at Ferndale, Michigan and was buried at Detroit, Michigan.
That the answers to questions propounded below are full, complete, and truthful to the best of my knowledge, information, and belief, and that no evidence necessary to a proper adjustment of all claims against the accrued pension is suppressed or withheld.
