Declaration For Increase Of An Invalid Pension.

County of Marshall

     On this 28 day of August A.D. one thousand nine hundred TWO, personally appeared before me, S. F. Earliwine a Notary Public within and for the County and State aforesaid HENRY ANSHUTZ, aged 62 years, a resident of MOUNDSVILLE, County of MARSHALL, State of WEST VA, who being duly sworn according to law, declares that he is a pensioner of the United States by Certificate number 360068, and duly enrolled at the Washington D.C. Pension Agency, at the rate of SEVENTEEN dollars per month, having served in the MILITARY service of the United States SECOND LIEUTENANT CO. B., 12th REGT West Virginia Volenteer inft and was honorably discharged, after a service of 90 days during the War of the Rebellion; that his present physical condition is such that he believes himself entitled to receive an increased rate under said act, and that he now suffers from DESEASE OF STOMACH AND RESULTING GENERAL DEBILITY, PILES, RHEUMATISM AND CRAMPS AS RESULTING THEREFROM AND THE CLAIMANT ASKS TO BE EXAMINED BEFORE THE BOARD AT BELLAIRE OHIO.

     That his Postoffice address is MOUNDSVILLE, County of MARSHALL State of WEST VA.

THE RECORDS AT THE COURT HOUSE OF MOUNDSVILLE WVA WILL SHOW UP EVERY THING NECESSARY. MY WIFE WAS CEDUSED BY AN OFFICER OF THE US ARMY WHILE I WAS A PRISONER OF WAR & HIS OATH IS REGISTERED IN THE ABOVE COURT.

Declaration for Pension

State of TENNESSEE, County of WASHINGTON, ss:
     On this 12 day of SEPT, 1918, A.D. one thousand nine hundred and EIGHTEEN, personally appeared before me THE UNDERSIGNED within and for the county and State aforesaid, HENRY T. ANSHUTZ, who, being duly sworn according to law, declares that he is 78 years of age, and a resident of NATIONAL SOLDIERS HOME, county of WASHINGTON, State of TENNESSEE; and that he is the identical person who was enrolled at MOUNDSVILLE, W.VA on the -- day of JULY, 1862 as a PRIVATE, in Co "D" of the 12 WEST VA. VOL. INFY. in the Service of the United States, in the CIVIL WAR and was honorably discharged at MARTINSBURG, VA, on the 20 day of MAY, 1865.
That he also served NO OTHER SERVICE OTHER THAN ABOVE.
That he was not employed in the military service of the United States otherwise as stated above. That his personal description at enlistment was as follows: Height, 5 feet 4 & 1/2 inches; complexion, FAIR; color of eyes, BLUE; color of hair, L. BROWN; that his occupation was FARMER; that he was born SEPT 19, 1840 at MOUNDSVILLE, WVA.
     That his several places of residence since leaving the service have been as follows: MOUNDSVILLE, W.VA AND THEN HERE TO SOLDIERS HOME, TENN.
     That he is a pensioner under certificate No. --. That he has -- applied for pension under original No. AND IS IN RECEIPT OF A PENSION, UNDER GENERAL LAW AT $30 PER MONTH.

     That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of May 11, 1912 as amended by Act June 10, 1918 at rate allowed by law.
     This his post office address is NATIONAL SOLDIERS HOME, county of WASHINGTON, State of TENNESSEE.

HENRY T. ANSHUTZ

H. T. ANSHUTZ, SURVIVOR OF LIBBY PRISON, DIES IN INDIANA

Capt. W. D. Alexander received word today announcing the death of H. T. Anshutz at Marion, Ind., on August 14th. He was buried at Marion.

"Tuck" Anshutz was well known in this city, having been born and reared here at the Anshutz family home on Water street. He was a veteran of the Civil war and spent many months in Libby prison at Richmond.

Mr. Anshutz is survived by three sisters, Mrs. Sep. Hall of New Martinsville, Mrs. Mary A. Collins of LaJolla, Calif., and Mrs. Rebecca Anshutz of Des Moines, Iowa.

The older people of Moundsville will remember Mr. Anshutz as one of the most popular men of this section.
